Omega-3 fish oil is not the same as omega-3: it is one source of two omega-3s, EPA and DHA, which fish take in mainly from microalgae. SACN set a UK population guideline of 450 mg long-chain omega-3 a day, and in 117,108 UK Biobank adults, the estimated omega-3 index was 5.58%. On keto, fish oil and algae oil both fit as pure fats; buy by EPA and DHA per serving, not capsule size.

What will you learn from the article?
Is omega-3 fish oil the same as omega-3? Omega-3 fish oil is not the same as omega-3: omega-3 is a family of fatty acids, and fish oil is one source of EPA and DHA. EPA and DHA are made by microalgae and other marine microorganisms at the base of the marine food chain; fish obtain them mainly from their diet and can synthesise them only to a limited extent, depending on the species [1].
The three omega-3 fatty acids you usually see on labels are ALA, EPA and DHA. Plant ALA converts poorly: in young women given 700 mg labelled ALA, net conversion was 21% to EPA and 9% to DHA over 21 days, while in 6 young men labelled DHA was not detected [2]. NHS oily fish examples include salmon, mackerel, sardines, herring and trout [18].
Omega-3 fish oil capsules, tablets, liquids and gummies can provide the same EPA and DHA, but they differ in how fast the oil oxidises once opened. In one study simulating consumer use, bottled liquids reached a peroxide value of up to 44.6 meq/kg and TOTOX up to 96.9, while capsules reached up to 7.6 and 30.4 [8]. The GOED industry monograph sets maximum limits of peroxide value 5 meq/kg, p-anisidine 20 and TOTOX 26 [5].
| Form | Maximum peroxide value in storage study [meq/kg] | Maximum TOTOX in storage study |
|---|---|---|
| Liquid (bottled oil) | 44.6 | 96.9 |
| Capsules | 7.6 | 30.4 |
| GOED voluntary limit | 5 | 26 |
Maximum values from one study simulating consumer use (Yenipazar 2023), not typical values; GOED is an industry standard, not law [8], [5].
Liquid suits people who dislike fish oil capsules, omega-3 capsules or omega-3 tablets, or who want larger amounts without swallowing several softgels. If you choose omega-3 liquid, follow the manufacturer’s storage instructions.
What do “high strength” and “1000 mg” mean on an omega-3 fish oil label? High strength and the “1000 mg” on the front have no UK standard. The mg figure usually means the weight of fish oil in the capsule, not the EPA and DHA. Read the EPA and DHA per daily serving and compare it with the SACN population guideline of 450 mg long-chain omega-3 a day [18].
Chemical form matters in the short term: re-esterified triglycerides were absorbed at 124%, free fatty acids at 91% and ethyl esters at 73% of natural fish oil in 72 volunteers over two weeks, and the differences largely disappear with long-term use [10]. Take omega-3 supplements with a meal containing fat [10].
| Chemical form | Short-term absorption vs natural fish oil [%] |
|---|---|
| Natural fish oil (triglycerides) | 100 |
| Re-esterified triglycerides (rTG) | 124 |
| Free fatty acids (FFA) | 91 |
| Ethyl esters (EE) | 73 |
72 volunteers, about 3.3 g EPA and DHA a day, two weeks (Dyerberg 2010); differences largely disappear with long-term use (Alijani 2025) [10].
What is omega-3 fish oil good for? Omega-3 fish oil is good for supplying EPA and DHA, which have authorised health claims. “EPA and DHA contribute to the normal function of the heart.” You need 250 mg EPA and DHA a day for that wording [20].
“DHA contributes to maintenance of normal brain function” and “DHA contributes to the maintenance of normal vision.” Each needs 250 mg DHA a day [20]. “DHA and EPA contribute to the maintenance of normal blood triglyceride levels” needs 2 g a day, and “DHA and EPA contribute to the maintenance of normal blood pressure” needs 3 g a day [20]. Combined supplemental EPA and DHA of up to 5 g a day raised no safety concerns for adults in EFSA’s assessment, but that is not a recommendation [6].
For omega-3 benefits on heart outcomes, effects are small and doses were from 1 g upwards: across 40 trials with 135,267 people, heart attack risk fell about 13% (RR 0.87), while total cardiovascular events were not significantly reduced, in a meta-analysis funded by GOED, the omega-3 industry body, and its lead author works for GOED [12].
Did you know? VITAL gave 1 g fish oil a day (840 mg EPA+DHA) for a median 5.3 years: no significant drop in major cardiovascular events (HR 0.92), but fewer total heart attacks (HR 0.72).
How much omega-3 fish oil per day? Count EPA and DHA, not fish oil. The UK guideline from SACN is 450 mg long-chain omega-3 a day [18]. EFSA’s adequate intake is 250 mg EPA and DHA a day [6].
For timing and amounts, see how much omega-3 per day. EFSA found no safety concerns for adults at up to 5 g EPA and DHA combined a day from supplements, which is not a recommendation [6]. The NHS advises two portions of fish a week, one of them oily, with each portion about 140 g [18]. UK adults aged 19 to 64 eat about 56 g of oily fish a week [18].
Did you know? NHS oily fish examples include herring (including kippers), pilchards, salmon, sardines, sprats, trout and mackerel; fresh and canned tuna do not count as oily fish.
Is omega-3 fish oil safe? Omega-3 fish oil safety depends on your dose and your health. In 11 trials with 120,643 people, bleeding was not significantly increased, with a rate ratio of 1.09 [14]. In large trials, the risk of atrial fibrillation rose with dose, about 12% at up to 1 g a day and 51% above 1 g, mainly in people with heart disease or at high risk; the analysis covered 8 trials and 83,112 people [13].
Contaminants vary between products and markets. In 37 Japanese fish oil supplements, estimated PCB intake was 0.00078 to 1.0 ng/kg a day compared with the WHO figure of 20 ng/kg a day; in best-selling supplements in Turkey, the limit was exceeded for lead in 3 products [4]. Oxidation also varies: 33% of 26 Australian products exceeded the total oxidation limit [7]. Human harm from oxidised oil is not established [7]. People taking anticoagulants or due to have surgery should ask their doctor; read more on omega-3 side effects.
Fish oil or algae oil: MEGA Omega 3 Vegan Algae Oil comes out best for keto and fish-free diets because it supplies EPA and DHA from Schizochytrium microalgae. BeKeto MEGA Omega 3 Vegan Algae Oil is fish-free, vegan, lemon-flavoured Schizochytrium microalgae oil in dark glass, with 570 mg DHA and 185 mg EPA per 5 ml [19]. In a 2025 double-blind trial of 74 adults, microalgal oil raised blood EPA and DHA as effectively as fish oil; the trial was run by an algal oil producer, with five authors employed by dsm-firmenich and the sixth a contracted statistician [3].
Cod liver oil adds vitamins A and D, so it needs a separate label check [6]. The algae oil used to make the product, before lemon flavouring, has a typical TOTOX value below 8, against the GOED limit of 26 [19], [5]. Do not exceed 5 ml daily [19]. MEGA Omega is one of our keto supplements for people who want a fat-based format. For fish-free and plant-based diets, compare it with other vegan omega-3 options.
“This product is intended for adults. Not for use by children or by women who are pregnant or breastfeeding. If you take anticoagulant medication or are due to have surgery, consult your doctor before use.” [19]

How is fish oil different from cod liver oil? Cod liver oil comes from cod liver and contains vitamins A and D on top of EPA and DHA. EFSA upper limits for adults are 3 000 µg RE vitamin A and 100 µg vitamin D a day, which is why cod liver oil needs a separate label check [6]. The NHS advises avoiding fish liver oil supplements such as cod liver oil when pregnant or trying for a baby, because they are high in vitamin A [18].
Looking for vitamin D on its own?
Fish oil from whole fish contains no vitamins A or D. Vitamin D3 drops and other keto vitamins are in the BeKeto shop.
Shop keto vitaminsWhat is the best omega-3 supplement in the UK? Choose the best supplement by EPA and DHA per serving, freshness and form, not by brand. Freshness means checking peroxide value; for flavoured oils, GOED assesses the finished product’s peroxide value and pre-flavouring p-anisidine, not finished TOTOX. Use our best omega-3 supplement guide for the full buying checklist [5].
Does fish oil help lower cholesterol? Fish oil does not lower total cholesterol. A meta-analysis of 64 RCTs found a fall in triglycerides of about 0.10 mmol/L (own calculation) per gram of omega-3 and no effect on total cholesterol [11]. DHA at 2 g a day or more raised LDL by about 0.19 mmol/L (own calculation) versus placebo [9].
Is it okay to take fish oil every day? Fish oil was studied as a daily supplement. Red blood cell omega-3 rose over 8 weeks in 20 people taking daily fish oil and fell after supplementation ended [15]. EFSA saw no safety concerns for adults up to 5 g EPA and DHA a day from supplements, but that is not a recommendation [6].
Does omega-3 fish oil help with memory? Omega-3 fish oil has no authorised memory claim. The authorised wording is “DHA contributes to maintenance of normal brain function”, with 250 mg DHA a day [20].
Does fish oil help with weight loss? Fish oil does not help with weight loss. In 11 RCTs with 617 people, the difference in body weight was 0.00 kg [17].
Is there halal omega-3 fish oil? Halal omega-3 fish oil depends on the certification shown on the label. Softgel capsules are often made with gelatine, and the source should be stated on the label. A liquid oil or an algae oil without a capsule shell avoids that question.
